Forms can be used to collect information about clients, such as their contact details, medical history, medical aid information and more. This information can be stored on the form itself, or can be merged with a client's profile on Bookem.
Information that is frequently required, such as the client's medical aid details, should be merged with the client's profile. This information is dynamic and will automatically be updated on the client's profile if they update the form with new details.
This merged information from the form will be accessible throughout Bookem's various functions and features. For example, medical aid details will auto-populate on invoices.
Information that is only needed occasionally, such as the name of the client's referring practitioner, can be stored on the form and need not be merged with the client's profile.
This information can be accessed from the Client "Forms" section of Bookem.
If you have all the required fields on your client profiles, then you can hop straight to linking them in your forms.
Here's how:
Go to Forms
Select the form you would like to edit. Click Edit
Search on the form for the field you'd like to link
Click Show More...
Click on the drop-down menu Link to client field
Select the client field you would like to link
Click Save
You have successfully linked an existing client profile field to a form field
If you don't yet have all the client fields you need, for example, you might want to capture Occupation or Medical Aid Details as well, you will need to add these fields into your client profile settings first.
This can be done by following these steps:
Go to the Clients section in Bookem
Click on the Profile Fields button at the top
Click on the + Add Field button or click the plus between existing fields
Select the type of field you would like
Enter the required information
Click on the Add Item button
Click Save
Once you have created a custom client field, you can link it to a form. To do this, follow these steps:
Go to Forms
Select the Edit button of the relevant form
Click on the + Add button or click the plus between existing fields
The profile field you created will appear in this list, select it
Save
Now whenever a client fills out the form, the information from the custom client field will be merged with their profile.
An example of how this works
You create a custom client field called "Patient ID #"
Now navigate to the form you'd like to use it on and click on Edit.
Then click on the + below or above any other field on your form
Choose the new client profile field (Patient ID # in this example) that you added in the steps above:
Click Save
Now when a client fills out this web form, their ID number will be merged with their profile. 🎉
